| Clarification | I filed the original D2 for the quarter ending on 12/31/14 correctly. However, when inputting amounts to file the D2 for the period of 1/1/15-3/31/15, the amount for non-itemized individual contributions for the 2014 4th quarter was incorrectly edited from $2,154.00 to $2,064.00. The change in the non-itemized individual contribution amount prompted me to file the incorrect First Amended D2 for the 4th quarter of 2014 before I could find the error as the electronic filing system (IDIS 3.0) required me to file the amendment before I could subsequently file the D2 for the 1st quarter of 2015. In this filed amendment I have changed the non-itemized individual contributions for the 2014 4th quarter to its original and correct amount of $2,154.00. This Second Amended D2 is exactly the same as the 2014 4th quarter D2 filed on 1/15/15. |
