| Clarification | On the original report in Schedule A Part 1 - Individual Contributions, we erroneously included three individual contributions as not-itemized. We are amending the original report to include the individual contributions of Quinn Costanzo, Dennis Fleming and Jesse Guevara as itemized individual contributions on Schedule A Part 1 and corrected the not-itemized amount reported on page 1 Line 1b. On the original report in Schedule B Part 8 - Expenditures, we reported an expenditure to Timothy Gallagher for reimbursement of cups, hand towels, openers and gloves for boxing event. We are amending the original report removing this itemized expenditure to Timothy Gallagher. Not one expenditure for these items was in excess of $150 therefore these expenditures are now reported on page 1 Line 8b as Not-Itemized. Also, on the original report in Schedule B Part 8 - Expenditures, we reported an expenditure to Michael P. Maloney for boxers, coaches, officials, announcer, doctor and paramedic, security, custodian fees for boxing event. We are amending the original report to list the vendors, organizations or persons from where the goods or services were purchased or provided. As shown on Schedule B Part 8 - Expenditures, the vendors, organizations or persons reported are Jim Burke, Team Chicago, Shannon Rovers and Thomas Treiber. The remainder of the expenditures originally reported to Michael P. Maloney do not exceed $150 individually or in the aggregate, therefore are now reported on page 1 Line 8b as Not-Itemized. |
